The Health Trust Family Resource Centers - Region 2

About

The Health Trust is an operating foundation founded in 1996. Our mission is to build health equity in Silicon Valley. We believe everyone in our community should have the opportunity to be as healthy as possible, especially the most vulnerable. To that end, we provide direct services, fund community-based organizations whose work aligns with our mission, and advocate for policies and initiatives that help advance our mission. One of the direct services we offer are The Health Trust Family Resource Centers, funded by First 5 Santa Clara County’s Learning Together Initiative and operated in partnership with Catholic Charities of Santa Clara County and SJB Child Development Centers. Our centers offer free programs and services designed to strengthen families and promote literacy, proper nutrition, physical activity and skills that growing children need to be ready to succeed in kindergarten.

Region 2 covers East San Jose and includes the following Family Resource Centers:

  • Evergreen FRC @ Katherine R. Smith Elementary - 2025 Clarice Dr. Room B5, San Jose, CA 95122
  • Rita Chavez-Medina FRC @ Cunningham Avenue - 1601 Cunningham Ave. Bldg D2, San Jose, CA 95122

Yes, multiple languages spoken at each site and some programs offered in other languages.

Yes, on a case-by-case basis. All families are welcome at our centers; we conduct developmental screenings and make referrals to other services.

All programs are free or very low cost.
